Medicare Annual Wellness Visits


Did you know . . .

If you've been enrolled in Medicare Part B for longer than 12 months and you're a patient of a Curry Health Network primary care provider, you're eligible for a free Annual Wellness Visit?

What is it?

We will ask you to fill out a questionnaire, called a "Health Risk Assessment," as part of this visit. Answering these questions can help you and your provider develop a personalized prevention plan to help you stay healthy.
Your visit may include:
Updating allergies, medications, medical and family histories
Reviewing prior immunizations and health screenings
Identifying out-of-date lab work, immunizations, or health screenings that you're eligible for under Medicare - all at no cost!
Vital signs, mini-cognitive evaluation, basic hearing screening, and depression screening
Receive educational materials on addressing Advance Directive Care

We will also perform a cognitive assessment to look for signs of dementia, including Alzheimer's disease. Signs of cognitive impairment include trouble remembering, learning new things, concentrating, managing finances, and making decisions about your everyday life. If your provider thinks you may have cognitive impairment, Medicare covers a separate visit to do a more thorough review of your cognitive function and check for conditions like dementia, depression, anxiety, or delirium.

What isn't it?

The Annual Wellness Visit is not a physical exam. It is designed to develop or update your personalized plan to help prevent disease and disability, based on your current health and risk factors.

Any additional tests or services received during the same visit are not covered by this Medicare benefit and there may be a cost to you.
