By answering a short series of simple questions, you can determine if you are eligible to receive a Covid-19 test from Curry Health Network.
If you meet the criteria, you may complete the Registration process online, and we will call you to schedule a time for you to have your specimen collected from the convenience of your vehicle.
If you do not meet the criteria to receive a protocol order for a Covid-19 test, you will be given the option to register for a Telehealth visit to see if, given your specific circumstances, testing may be right for you.
If you have problems with the online process, make sure that your file attachments do not exceed the maximum size, and that you are using an up-to-date browser.
If still unable to submit, please call 541-412-2000 (Monday-Friday 7 am-5:30 pm, excluding holidays) and let them know you are unable to register via the online form. If you call outside of business hours, your call will be returned the next business day.
